Yeah, it's offensive. It rates right up there with using any other ethnic slur as a negative pejorative. What do I do about it? I correct them, directly in front of whoever happens to be there.

Well it is. Using words like these are not politically correct anymore and could certainly offend ppl who are gays. Just like so many blonde jokes may quite be a damper for a blonde person.

It was originally meant to be, and those who insult others like that are homo incognito. But then again, my best friend is gay and he says things like, "OMG, that is so gay" lol
So no, it does not offend him.

Personally, I don't get mad the first time; I get annoyed. And I correct the person. From then on, they know it bothers me, and if they continue then they know full well it's going to piss me off.
