Put vaseline on the floor where she will most likely walk and she will slip and fall, might be to harsh considering she could hurt herself.

You could ask a couple of guys ( that you know and trust but your friend doesn't know) if they will help you on a prank I usually go for the really well thought out and planned out kinds of pranks have them fake that they are trick or treating at your house and when you two open the door have them flirt with you a lil bit and then invite them in and after a while make them act as if they are homicidal maniacs that are out to kill young girls, have them make it seem like they took you to the next room and killed you, put fake blood on your throat and have them bring her into the room. That will definitely freak her out. Or just put saran wrap on the toilet. its easier and doesnt take too much time.

It's a good prank, though not scary, to coat a bar of soap with clear nail polish. Put that one by the sink she's likely to use to wash her hands.

You could always cut up tiny scraps of paper and put them in a lightweight plastic bowl. Balance the bowl on the top of a partially-opened door and ask her to go get something for you in that room -- she gets a confetti shower.

Sooooo many things ran through my mind. It depends on how easily she scares.

Hand in candy bowl.
Hide & jump out.
Have someone else dress up and do the scaring (she might be expecting you to do something).
Play a movie that YOU have seen and when the scary moment arrives - grab her arm and yell.
