At most universities, faculty are absolutely prohibited from entering into relationships with students, both graduate and undergraduate. This is because of the power differential. Faculty members constantly evaluate graduate students (even if they are not their advisors), and relationships could alter the fairness of this process.
In my experience, serious relationships rarely happen because, well, graduate students are "works in progress," rather than full colleagues. Few professors would even think about a serious relationship with a subordinate, without intellectual, emotional, or even financial equality, especially since it would jeopardize their careers. Even making advances toward a grad student could result in a sexual harrassment charge, and endanger the professor's career.
However, I must add that despite the prohibitions and dangers I've mentioned, I have often seen male professors flirt with or "hit on" female graduate students, just to see what they can get away with. Would I call this a "crush"? No, I would call it lust, usually in the context of a midlife crisis. And in most cases, the women (wisely) rebuffed these men, since it would also hurt the women in the long run. (Their peers would censure them, and they'd probably worry about whether they were being unfairly favored or unfairly criticized, etc.)

yes it happens, and is typically forbidden, especially when the professor has some kind of authority over the grad student (such as being his/her faculty advisor). However, exceptions do exist, and some schools/programs are more lenient than others. Indeed, there are many instances where professors have gone on to marry their graduate students (typically after the student has left the program).
