If you have an ex-spouse, that person would normally be the "ex". However, if you were married long ago and have since had another serious relationship and/or a relationship that produced a child, that boyfriend/girlfriend would more likely be the "ex". I have also found that people who use the expression "my ex" tend NOT to be over the person (whether they were married or not).

It can be either. Before I'd been married, I thought about it as ex-boyfriends. After having been married and divorced, I think of ex-husbands when I hear the term 'Ex'.
Basically, it's a matter of perspective & can mean either way.
