Would my information be on the phone like my name, how many minutes a month, and what plan I'm on

if you reported it stolen to your phone company then no. cell phones are tracked by their esn#, and as soon as you report it stolen, that phone is red flagged and no one can reactivate it.

Honestly, it depends on what you mean by "activate".

If you called your service provider and suspended your service, then it can only be reactivated by someone who knows the name on your account and the last 4 of your SSN.

If the type of phone you lost is a GSM phone, then the person who found the phone can use the phone, as long as they take out your SIM card. They can even change the serial number for the phone (this is illegal, but CAN be done) so that way, if they ever decide to register the phone, they won;t get caught.

But don;t worry, they can;t use your service.

No,not unless they know you're acount info like you're name, what plan you're on and how many minutes you get per month because no one should know that info but you or close relative. Sorry it got stolen :( Cheer up though. :) ;D
