You're only allowed to give out hire date, leave date, and if he's eligible for rehire, so I would just do it, but if you really don't want to, just say that you're not comfortable doing that.

With complete honesty. Tell them that you would only be able to give a reference that reflex their actual job performance and ability. Let them know that you are not comfortable in doing so and if they think they deserve a reference from the company, they need to talk to there old manager, supervisor, etc.

You could say you don't feel qualified to be his reference since you were only his coworker. If he really presses you, you'd have to fess up that you thought he stunk as an employee.
