A few actually
I was messing with my kids one day and we were sticking Banana stickers on our foreheads, the kids forgot to tell me I had the sticker on and I went out to our local grocers came back and they said oh mum u didn't go out with that on ur head did you, talk about embarrassing
the Second was, I went out once with the same pair of shoes on but different colours, I had a blue and a black pair, i want out with one blue and one black, didn't even notice till I got back and took them off, how embarrassing is that one?
The third was, when I left my mums house and walked down the drive got in my car and got home, went into my house and realized I had left my shoes at my mothers house, I had to go back for them imagine how the house was in uproar when I walked in and my mum was waving them in the air at me, and said forgot something Tracey. That one was very embarrassing.
Take your pick, to me all 3 were very embarrassing, I never done them since and never will, but no doubt I will do others to add to my little red book of embarrassing moments, lol

When I was a little kid, I was playing walkie talkie with my little brother. This was over 25 years ago, when they were not very good. So I had to move in different directions to get reception. I finally got great reception beside my bedrooms radiator. The reception would come and go, but would always stay if I kept my head close to the radiator.
I ended up getting my head stuck between the radiator shutoff knob and the wall. My dad had to take the knob off to get my swelling head loose. To this day, my family mentions this when they see me around a radiator.
This is not my most blushing story, but there is no way I am putting that on the web.

When I went to turn the car on it wouldn't start and I couldn't figure out why so I kept trying and I realize I left the car in drive...I felt really stupid lol
and there was the time when my friend was at my window while I'm hanging over the window to get something from him he started laughing at me and I asked him whats so funny he was like "probably because your boob is showing" (I didn't realize it) :( since then I made sure I cover up my chest more often lol

Quite a few...but most recently, my son, who was 7 at the time, was outside with my husband & I visiting our neighbors...one of them is an Indian, although does look a bit oriental (very, very nice & quiet guy)
So my son, being so honest as he is, blurts out, "Hey, he looks like the Chinese guy on the show Mad TV!"
We all just pretended it didn't happen, then later my husband & I spoke with him about it.

I am a senior in high school and i am and aid in a class with ONLY freshman boys(one of them just ahppens to be my brother.... well while the teacher went to the office i was at the board helping the students with their work and my little brother pantsed me(pulled my athletic shorts down) and i was wearing a THONG!! the whole class full of boys LOVED it!! lol good thing i have a nice ***!!! i was SO embarrassed i beat the crap out of him when i got home!
