And why does it seem to occur on the forehead?

Just to be clear - I don't mean a discolored spot, it's usually the same color as one's skin, but raised like a mole.

this is not a mole , it is some sort of hyper pigmentation in the skin that happens because of too much exposure to the sun. it increases as you age . that's why the beautician always advise us to use SPF not less than 15

Probably. It's probably from sun exposure, which would explain the forehead. I've heard that after 30, you longer get "freckles", they are considered "age spots". That is seriously depressing.
