Practice, practice, practice.
Find a native English speaker and work on your English daily if at all possible. If you have mastered the fundamentals but are concerned about your accent, work on pronouncing important words you will use every day with a native speaker who will correct your mispronounciations.

If you are in India, read The Hindu newspaper daily. Follow their Know your English column every tuesday. List unfamiliar words and check with Longman dictionary which gives usage as well as meaning. Practice with friends so that you don't have to worry about mistakes.

talk to people with good english background such as your co-workers, friends, teachers; practice on how to pronounce words that are difficult to you, reading books and watch the news to hear how the people speak on there.
first of all just learn the basic rules of grammar like tenses and verbs and like wat verbs should be used in which tense and be sure to learn pretty much all rules of grammar and then try to apply them by constantly speaking in english.A good trick is to forget that u can speak u r mother tongue and just think in u r mind that english is the only language u know and even when u speak or think in u r mind u must make sure dat u undergo dat process in english

Start an online group with headphones and microphones, and offer to do some programming or something for someone in the United States to compensate them non-financially, for their time to help you with accent reduction etc. I think you would have fun.
You could probably have 10 students in the room.

Practice. Just like with anything you do, you have to practice. Jackie Chan learned by singing with Willie Nelson music, and I once met a Japanese man who said he learned all of his English from Mel Gibson movies.
