For TV shows...they make money by you buying the stuff either subconsciously used in the show or the products used during commercials. They advertisers pay big bucks for most watch shows on TV.
On the Internet, web sites sometimes do get paid on a per access or per click basis. For example, if you click on the advertisers on Google (the ones on the right side of y our window or the one at the top in blue, that website pays google a certain amount of $ for the click to go to that site.Its called a referral page.

Yes, it is rated on viewers. This is how they know if a show or site is good by how many viewers they have. Us as viewers make them more money. So, the longer you watch a show on any station, the longer they keep it going. Same for a website.

TV is judged by a set of boxes in homes across the nation that monitor viewers if you don't have one then you don't count ( they guess at the overall number based on the box homes that tune in - THE NEILSON RATINGS )
WEB most sites get their money when you click on an ad but they also sell ad space based on the number of visitors - based on the other answers the new policy is to BURY the other answers and hope your wrong answer is what's left ( lol )
