It depends. I lived in Italy for two years, and the women were absolutely gorgeous, that is till they hit their late fourties, then they start hunching over and getting really hairy.
One good frame of reference is to compare famous Spanish women with famous Italian women (sorry, I'm using women because I'm a guy, but you can do the same thing with men).
Jennifer Lopez vs. Sophia Loren
Salma Hayek vs. Isabella Rossellini
Daisy Fuentes vs. Valerina Golino
Of course it's just a matter of opinion, but I say Spanish.
Hmmm...here's some famous Spanish vs. Italian guys:
Antonio Banderas - Al Pacino
Jose Ferrer - Rudolph Valentino
Ricardo Montalban - Robert Deniro
Tough call; I'd say it's pretty even.

Not a good question. Any answer is going to be based on a biased opinion. Beauty is in the eye of the beholder... There is no basis or quotable source for saying definitively that one is better looking than the other.
Now my biased opinion is that I have never met or seen a good looking Spaniard. I am an Italiano and I know I am damn good looking so there you have it... Italiano's are much better looking than Spaniards!
