The large urban community hospital may be seeing more of the level one traumas or cardiac events and less serious events may have a delay before they are seen

I believe they would also have more interns and students per patient then a small hospital or a non community hospital. It helps keep down the costs and allows the next generation of health care workers to grow in their practice, but it also means the first people who directly give you care may have less experience.

The patient population of the ER waiting room may be sicker (read more contagious) because the service is used by people who have no insurance as a primary care facility when they get so very ill that they must seek attention or do not realize they are very ill (read drug users) until they are sober enough to notice

Human touch is absent.
It is costly
Not convenient for villagers
You have to pay for anyhing even when it is supposed to be free
