i saw a costume at a halloween store, but i'm sure you can easily make the same one at home. it was a guys costume that came w/ a white doctor's coat and a box for the head which read "mammography machine. place breasts here" and it had a cut out with 2 half circle shapes where the guys face goes.

I did this a few years ago - it worked really well:
A Gynecologist.
White lab coat, scrubs, head lamp (I used a camping headlamp) - you know, the normal doctor outfit...but for added a few extra props:
Biggest tube of KY Jelly I could buy
I took little kids soccer shin pads, wrapped them with aluminum foil and attached them to the shoulders of the lab coat - AKA Stirrups.
Metal Salad Tongs to get a "good look inside - AKA Speculum
Bottle Washing Brush to get a sample for the Pap test.
And of course, a pocket of condoms

Dress up as a screw driver or screw and have a sign that says "Wanna screw me?"
Be a lolly pop that says suck me.
