A different idea,
since you love him, go get his parents (or close family members and bring them to your house (after treating them with proper courtesy) and explain to them, how much he loves you and has been good to you. It will all create a positive effect on him, that he has been useful to all of you.
Then all of you go on a family outing, on a weekend.
I bet it is something he will remember. Because getaways with relatives and families are so rare today, with each people having their own egos.
In addition a good gift is a must. If he likes magazines, an yearly subscription for one of those is a must.
OR
Also even older males like video games, an XBOX is a good. We males think it is only for kids but given a chance, we will want to do it all if we are given a chance to be kids

Sex is always a good gift. Is there some fantasy that he's been harboring that you could act out with him?
Also if he's into sports, get him a collectible from his favorite team, or tickets to a game.
Tools are great if he's one of those handy-man types. I gave my husband a gift certificate to Home Depot and he LOVED it! He was able to pick out just what he'd always wanted.
If you have kids, alone time is also highly appreciated because it's so rare. Get a sitter.

You can make a dvd or cd of pictures of you and him from beginning to present set to a favorite song that you share. Or set pictures of a special occasion to music. That is the new technology that pricks the heart. The more personal you make it will just sweeten it that much more.

is he a fun loving guy ? serious ? wacky ? go with his personality and have a party... games, friends, family, etc....... dance maybe ?? something that puts the spot light on him for a change and shows him you appreciate all he does...... speaches, a roast, like they do for the celeberaties...... have a money tree instead of gifts, make sure you say that on the invitation or tell verbaly, so he can buy what he really wants.......
