Yes. It is advisable to request for an appointment letter even though u are a temporary staff. This is to safeguard your own interest. The appointment letter should state your salary, benefits, duration of notice to be given prior to resignation, term of your employment (how many months), etc.

yes u should..to safe guard yourself and for the company..it should state how much is your pay..any benefits..or OT..etc..if resigned..need to give how many notice?

yes because if their is a dispute over pay or hours or lenght of assignment you need to get it sorted at first
