I don't think there's a best designer and model. Each designer has their own signature style, so you can't really rule out which one is the best. There's a designer famous for couture, others for ready to wears, others for suits and so many things. Bottomline, depends also on the person choosing a wardrobe on which he/she prefers. That's why designers are there, to make sure that clients will wear something from their preference but not comprimising their signature or what they are known for in the society.
When it comes to models, I don't think there really is the best one. Models are nothing still without designers. Besides, photographers can really do a lot of manipulation now on photos just to make the model look that great and professional. Ever heard also of make up (some of them are not really that good looking), thats why thank god for make up.

i like classic designs like : Prada, Halston, Oscar Dela Renta, DKNY, Armani, Micheal Kors,
best models: oldschool - Beverly Johnson, Naomi Campbell (before the abuse), Vendella, Cindy Crawford, Linda Evangelista, Rachel Hunter, Brooke Sheilds ( back in the calvin kiein days)
male models: Tyrese & Tyson Beckford
