Does anyone know where I can find free anime/vampire/therian id cards that can be sent to you in the mail?

I was wondering because I saw something similar to this hanging on the inside of a friend's car, and I wanted one for myself. On the left side it had a drawing/picture, and on the right it had info you fill in yourself.
