Passport, and all of the travel arrangements. US citizens have an automatic 90 day tourist visa. When you enter the country they will want to know your hotel and flight information. Just show your passport and write down the information on the entry cards. It is that easy.

You don't need any shots! If you qualify for an American passport, all you'll need is your return plane ticket! They'll stamp your passport when you arrive, and that's your tourist visa!
However, if you aren't eligible for an American passport, you should check this list;

Your country of nationality is the important thing - NOT the country where you happen to be right now! You can look for your country on the list, and if it's there it means that you don't need a visa for a tourist visit. If your country is NOT there, you'll have to take your passport to the Japanese embassy or consulate to have a visa attached. This could take some time, so phone first for all the details of what documents you need to take with you, and ask how long it will take! Don't book your ticket and THEN apply for a visa, apply for the visa first!!!!

A valid passport and an onward/return ticket are required. Passports must be valid for the intended period of stay in Japan. A visa is not required for tourist/business stays up to 90 days, as long as you don't work in Japan.
