Can the immune system really mis-identify a fertilized egg as a pathogen, and is there a treatment to prevent this?

Yes. There may be other conditions but the one that I have is elevated natural killer cells. These cells are normally the body's defense against cancer. In my case they behave abnormally and mistake the embryonic cells for cancer cells and attack them. This can cause repeated failed implantation (so you would never know you were pregnant) or repeated miscarriage.

Treatment is a blood product called IVIg. It helps to normalize your immune response so that your body does not attack the embryo and allows you to carry the baby to term.
