reactions cause matter to change, temperature, pressure catalysts can affect reactions. the frequency of the interactions in the molecules or particles are changed therefore creating phase changes.

Take H2O for example When the liquid is heated up above 210 degrees to it turns to steam, when it is frozzen below 32 degrees it turns to ice and when it 32 degrees or more the water remains in the liquid state. The space between the molecules is the only thing that changes within the matter. When in the gas phases the molecules have a lot of room to move, when in the solid phases there is almost no room to move.
