i have two ideas for costume ideas based on nicknames, but i don't know how to do them
Miss Take (mistake) and
Katrina and the Waves
any suggestions?

it has to be miss take because my friend's dad always says "hello, miss take. i mean miss kate"

For mistake you could wear some mismatched clothes/socks/shoes, do your hair and makeup really wild. It is the one night a year you can get away with it! =)

For Miss Take you can have a name tag that says Ms. Take on it, and you could be wearing a shirt and hat on backwords. You could possibly even dress up as a medicine bottle by cutting a trash can untill it has arm and leg holes, then put directions like 'TAKE one every hour'.
For Katrina and the Waves you could dress up in a blue outfit with waves drawn on it. You could try gelling your hair in a wave shape and dying it blue if your really going for it. Then you can wear a name tag that says Katrina on it.

You could try Miss Steak .... but not too sure how to do this one either. A cow costume maybe?
