Lowercase L (l) and O (o) are often confused with 1 and 0 respectively. And the letter e would be confused with e, the natural logarithm (2.718281828...)
So I would say L, E and O.

I wouldn't say any letters are not used ever.
But many good answers appeared here to say not o or O and l or I (el or eye) as they look like numbers.
Also "e" is a good one to avoid except when it is meant as Euler's number. Which means it does get used!
L gets used as the name of the Lagrangian.
i, ,j, k, l, m, n all get used as indices or integers
u, v, w, x, y, z are used as variables
a, b, c, d get heavy use as constants
f, g, h are used as function names
o, p, q, r, s, t get their share!
There really are not many good "never used" letters!
