No. Because HIV status is such a loaded issue very few doctors will do that test without your explicit consent. Meaning, you will probably have to ask for it, or your doctor will recommend it if they think it is necessary.

Not on a routine basis, no. You have to ask for it or they might recommend it based on your personal risk profile.
New recommendations I've seen headlines about recently suggest that more of us should be tested on a more routine basis, so that might be part of the future of the routine gynecological exam. But it's not now.

My doctor does it when I have my yearly blood test even for my 80 year old grandmother because even though my grandfather is died there are other ways of getting the viris.

They may if you inform them that you are part of a high risk group that is an IV drug user, has slept with an IV drug user, has slept with any men who have had sex with other men.
Other than that you'll probably have to ask for it.
