The first and usually easiest way is to contact your employer and ask for a reissued W2. They should get it to you, but make sure to ask how (in the mail, in your cubby at work, in your paycheck, etc), and when so that you can keep track.
If you worked for someone who is no longer in business or who you cannot contact, you can get a transcript of your tax information for FREE from the IRS. You'll need to file a Form 4506T to get this. I've attached the IRS link for your convenience. The transcript that you get does not show your actual W2, but does contain all the information necessary to file a tax return, including SS and Medicare taxes withheld and the EIN number of the employer.This should be able to be used in place of the W2 in all instances - except for state taxes. The Transcript does not contain State tax information, and without knowing what state you live in I can't help you with getting that info. Your employer has to supply it for you. That assumes you didn't file a tax return in 2004, in which case you could get a copy of your 1040 from the IRS.

Call the INS they will ask for your Social Security Number and a few other things and then mail you a copy.
