old tiles need to be removed, new tile put in and re-grouted

If you are just talking about replacing a few tiles and not the whole wall then you first need to remove the grout around the bad tiles and score the bad tile and carefully break it with a chisel. Once removed, clean out the old adhesive with a scraper. Put in new adhesive, put in new tile and once dry, re-grout.

It depends on what the old tiles are attached to. If it is drywall, tear out the drywall and the ceramic tiles in pieces you can easily handle. Use a "saws-all" along grout lines and cut thru the drywall. Then put up cement board in place of the tile. Use thin set to stick the tiles to the cement board.

If the tiles are on old-style lath and plaster, use a mason's chisel and hammer to separate the tiles from the wall.

take a big hammer and smash them out and redry wall the area with proper dryway made for bathrooms and then no drywall compound just tile cement and tile and grout it. get a tile cutter and make sure tiles are square and where you want them. have enought tiles with some extra's for mistakes. by replacing the dry wall you see if there is any dry rott and can put in more insulation and keep more heat in there

I would take a small chisel and hammer and break away the grout, then tap the tile to crack and pry out. Clean well and prep for new tile. Be sure and wear glasses as the tile splinters.

Pneumatic drill and heavy duty sander.

Just be careful to don't drill right through the wall like the idiot who did our bathroom ...
