A location that's easy to find and easy to get to. A place where it's highly visible from the road. A location where there won't be a traffic jam, but where a majority pass by. Please go to the source link I provide, it has some good info in it.
I don't think there's a best state to have a restaurant. You can have a restaurant anywhere, what matters is if people go by the location. I would find a place where there's continually growth in the city or town.

Well, at least one thing is to make sure it's on a two-way street. I often pass by restaurants that are on the other side of a divided road since it will take too much effort to get to the place.

I can't say. There are so many things to consider. Cuisine, clientele, costs, location.
I did it for 9 years in a small town, and while the food was what they wanted, they wouldn't consider trying something new. And it's damned expensive to run a restaurant.
