Gaining a better understanding of their illness, injury or disability.
How to maintain and gain fitness levels, taking into consideration limitations.

I work at a children's hospital and I had the difference explained to me this way:
Occupational therapy generally helps with fine motor skills -- things you typically do with fingers, hands and arms. Brushing teeth, eating with utensils, writing, etc.
Physical therapy is generally about regaining gross motor skills -- mostly with the lower body/legs. Walking, climbing, running, jumping, crawling, etc.
