YES A BOWLER CAN BE CHANGED DURING HIS SPELL ,IT DOES NOT MATTER IF IT IS LAST OVER OR 45TH OF 10TH.
IN ANY OVER IF A BOWLER IS INJURED AND UMPIRES AND FIELDING CAPTAIN THINKS THAT HE OR SHE COULD NOT CONTINUE THEN A BOWLER IS CHANGED.ANY ONE MORE THING IT`S A MUTUAL UNDERSTANDING BETWEEN UMPIRES AND BATSMAN AND FIELDING CAPTAIN,WEATHER TO CHANGE THE BOWLER OR NOT.
BUT IT IS VERY MUCH CLEAR THAT IF BOWLER IS INJURED HE/SHE CAN BE REPLACED.THATS ALL MY DEAR FRIEND I KNOW.HOPE U CAN GET BETTER ANSWER WITH ICC RULES FROM ANYONE ELSE BUT THIS IS WHAT I KNOW.
BYE HAVE A NICE TIME AND IF U GET ONE BETTER LET ME KNOW ON jota_don@yahoo.com

Yes he can be replaced by any bowler who has not completed or bowled the over before. Last season they tried out the super sub rule, that they could change one play midway through the game.
I think both captain of fielding side and umpire. They might have to bring on a part time to finish the rest of his overs

Yes. The rule permits that if a bowler get injured in the middle of an over by another bowler, who has not completed his quota of overs and has not bowled the previous over. He can also not
bowl the next over.
The rule is reproduced below for your information:
"If a bowler is injured during an over and cannot complete it, another bowler must bowl the remaining deliveries in that over. The bowler chosen to finish the over must not be the bowler who bowled the previous over, and must not bowl the over immediately following either."

Yes, any bowler can be replaced if he is injured doesn't matter in the first over of the innings or last over of the innings.
Provided, the bower who is going to finish is over is not done with his quota.
