In answer to your question it takes time to see improvement in anyone and it does not happen over night. Not only will the counsellor be aware of this so will the teachers.
The way that they will be able to tell is if the student is a lot less stressed in their work, not asking for as much support or help, that their work has made some improvement and not in such a muddle. They will obviously be monitoring work and contents to see if there is improvement. Time management that you are more organised and things are not getting on top of you. That you are generally happier than you where not only inward but outward as what you look on the outside your apperance is also a good indicator that things are better.
All counsellors and teachers are well trained in the pyscological effect on students with work and know what to look for and what questions to ask. It is a combination of things not just improvement in your work, attitude,appearance, social skills there is a lot more to it.
I think that is the response you are after from your question well it is the way I have seen it. Your tutor/teacher.counsillor will have a good idea on your progress and also remember the very important rule they are not pyscic if you are finding to much you must tell them. Knowing what they look for is not the answer to make you what ever it is you want to look to them as in the end the only one you are letting down is yourself so please tell someone if you are finding it difficult.
Good luck course work is hard my daughter has just finished hers and now starting again with college.
