A few years ago, I had to travel with my cat from Michigan to Washington. There were two options (this was on United Airlines, other airlines may vary).

1) She had to be put in a box that was small enough to fit under the seat in front of me. She could not be removed from the box while at the airport or during the flight.

2) She had to be checked into the cargo area (usually separate from the place where you check in your luggage). If you go this route, weather conditions from the starting point to the ending point have to be such that the animal can survive. Meaning it can't be too hot and it can't be too cold. It's nearly impossible in the dead of summer or the dead of winter.

I took option number two and lucked out one day in June. I had to wait about 5 extra days though, because the heat was too much. There are also companies that outsource this kind of thing...particularly because it is hard to be there with your pet on the flight and not remove it from under the seat. :(

Normally air lines will only accept live animals in the cargo hold. There are exceptions, some Middle East airlines will allow Falcons in the cabin which becomes a bit hairy when one hits turbulance. As these birds are expensive their Owners are usually extremely rich, so me who flies "cattle class" dont have many close encounters with these birds.
Best carriers that I know of for looking after live animals is KLM.

That would vary by airline. You have to research the particular airlines policy. If allowed they will only allow one or two pets per flight so you have to check that out too.
