Where have I gone wrong? I know I have because I checked on the calculator but that doesn't explain anything.

You started the problem correctly, and your mistake is in the 4th line.
The correct solution goes like this:

69/8 = 8 r5
8/8 = 1 r0
1/8 = 0 r1 (Note that when you divide 1/8, the quotient is 0 and the remainder is 1)

So 69 = 105 base 8

69 div 8 -- > R5 and 8
8 div 8 --> R0 and 1

So the answer is

105

Check the calcualtion by

1 * 8^2 = 64
0 * 8^1 = 0
5 * 8^0 = 5

Total = 64 + 0 + 5 = 69
