You will need:
1. Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO)
2. Document indicating fullname, date and place of birth and citizenship (e.g. valid employment's ID, voter's affidavit, NBI clearance and driver's license).
3. Three (3) copies 4.5 cm x 3.5 cm new photo (colored with plain white background). Photo should show applicant in decent attire with collar.
4. Personal appearance except when the applicant is:
8 years old and below;
65 years old and above; or
mentally or physically incapacitated
5. For married women, bring authenticated marriage contract

First if your single you need to bring your original authenticated birth certificate,original baptismal certificate and 3 other supporting id's that were issued 3 years before cause that will be their basis that is what was really you,passport size picture's with white background and you should wear tops with collar then you can go to dfa to file and fill up applications there,but if you will be using a married status you should bring your authenticated marriage certificate in addition to the first documents i have mention.

birth certificate, 2x2 picture, just in case bring 1x1 and that's it. if you're getting for a kid, bring the mothers passport too.
