This sounds like pop-eye, and fin rot. The #1 cause is poor water conditions brought on by not changing his water often enough, or over feeding. What to do:
0)Change his water, and clean his tank/bowl. Be sure that you dechlorinate the water, and that it feels the same temp to your finger. (Rapid temp changes to sick fish are often lethal.)
1)Get a strong antibiotic. Melafix/bettafix is great if it's a minor case of finrot, but pop-eye is a different story. What you want is trisulfa. If you can't get that something with ampicillin works well. If you can't find either melafix/bettafix may work, or the combination of marcyn, and marcyn II.

Do a 50% water change. Add 1 teaspoon of freshwater aquarium salt per gallon of water. Test your aquarium for nitrates, nitrites, and ammonia. Make sure his water is staying at a stable temperature and that the temperature is between 70-80 degrees. Then look on the internet and try to figure out what the specific problem is. It sounds like fin rot, but it could be body fungus. If it is, both Jungle Labs Fungus Eliminator and Mardel Marycin and Mardel Marycin 2 should help. The most important thing though is that he has clean water free of ammonia, nitrites and nitrates. The freshwater aquarium salt acts as a sort of de-stress tonic that could help him heal. And the medication is incase clean water and salt aren't enough to heal him.

Get BettaFix from your pet store. My betta was having similar problems. After 1-2 weeks of putting the drops in his water, he was all better again.
Yours sounds like he's far along, but it may not be too late yet.

check out this web site. but change the water, and get some medication that is for fin rot!! do it fast. also get some freshwater aquarium salt this will bring down his stress level. but get the medication and fast he could die from this. the meds should be at your local walmart. look for a little bottle that says - fin rot. the aqua salt should be there too

your betta probably has tail rot or something. and if your bettas eyes are like popping out [both or just one of them] it probably also has popeye too. you should fix ASAP!!!
