No one knows why, but he had an infarction in his leg, similar to a heart attack. To save his life Stacy acted as his proxy and okayed a surgery removing part of his leg muscle. There is some dispute as to the actual long term impact. House started taking Vicodin for the pain, but he did almost no physical therapy. It is unclear how much pain he really experienced as opposed to how much he became addicted to the pain killers.
At the end of the second season, he was shot. Cuddy came up with the idea to use the opportunity to give him a new drug that would "reboot" his brain so that he wouldn't feel the pain in his leg. Less pain would mean he could exercise and begin to rehabilitate his leg and kick the vicodin habit. It seemed to work and he was happily (yes happy House) running to work when the third season began. Either the effects wore off or House started to feel sorry for hiimself, but he has started to experience pain again and to use his cane again. There is just enough ambiguity in the reasons behind his leg pain to keep his character interesting.

Short version: He was in a bad shape and while he was he didn't want to have the certain procedure he was afraid he would lose the use of his legs. But when he went "out" his wife talked Dr. Cutty into doing the procedure and something went wrong leaving him with a limp and on pain killers for the rest of his life. And when he got shot he asked Cutty to give him a drug that helped him walk for a while but eventually the pain and limp came back.

Because the infarction blocked distal blood flow, it was possible that House might require amputation of his right leg, below the level of the damage. .....

He was a drug addict. He had a pain in his leg. He didn't want it amputated and waited to long for surgery. He kept his leg but because of it he has to use a cane and take a lot of pain killers.
