Because Al Qaeda have never been a major component of the insurgency in Iraq. The insurgency is homegrown, the same as it would be if someone invaded our country and tried to steal our national resources. The same as if someone had invaded our country and started building a palace (calling it an embassy) on over 100 acres of land in the middle of DC. The same as if someone had caused the death of nearly 600,000 Americans. The same as if someone invaded our country and said they were going to give us democracy, but then overthrew a democratically elected government because they didn't like who we elected. Should I go on?

An "insurgent" is a person (generally non-uniformed) that attacks an opposing combatant or militarily significant target in an effort to further his/her cause for the purpose of reducing the opposition's ability to fight.
A terrorist is a person (generally non-uniformed) that attacks a civilian or non-militarily significant target in an effort to further his/her cause by invoking fear in the general population.
So, when you see "...IED set off in a market..." that's a terrorist. When you see "...shot down a US helicopter..." that's an insurgent.
You'll have to keep count yourself, because the news agencies will count and report based on their agenda.
As for counting Al Qaeda, they don't carry IDs, so it is tough.

When the US invaded Iraq, Osama said that the Iraq War was something that could not be lost by the Taliban. So, many of these insurgents are probably Taliban operatives

There's very few of the insurgents that are not Iraqi citizens, way less than 10%. You don't hear of them being killed because they are not many there.
