If the queen had two balls (testicles), she'd be king. It's a comment on the different treatment of the sexes, and how arbitrary it is.

The king may be given more respect than the queen, but if the queen just happenned to have a pair of testicles, she'd be the king and have the same respect.

This saying has been around for YEARS! My mother said her grandfather used to say it so that was 1930 and 40's. While I am not sure of its lineage I would bet it came from the time when women needed to go to work during WW I and WWII, since many men were serving in the military. It was a way for someone to state that even though she was a woman, she was working"like a man" and if she had 2 testicles she could be a man.

The queen says "Balls" as an expression like "Damn" or "Shoot." The rest is self-explanatory.
