In modern times, Yemen. Yugoslavia (South-Slav-ia) fell apart, but lasted for a while.

The Dutch did it by themselves after throwing off the Spanish yoke.

China and India are really multinational, multi-ethnic, multi-lingustic imperial states. Pakistan is unified only by relgion, and not ethnicity or language.

The US, Canada and Australia probably qualify, as the option for independence for the various states/provinces was a possibility at the time of unification.
