I kinda suspected that Carmen might really be Sheila but I guess we'll never find out now. I know they said Sheila was having plastic surgery to make herself look like Phyllis but who's to say that wasn't a ruse to fool folks into thinking that Sheila was dead? Of those who wanted Carmen dead, I'd say it was Brad. He's already killed before so it wouldn't be any big deal for him to kill again.
In any case, the new writers for Y&R are ruining that show, imo.

Considering that Lynn Latham ("Knots Landing," "Port Charles") is running the show now, I doubt that even the writers themselves know.
Latham is famous for making up stories as she goes along (which led to some serious continuity errors on "Port Charles.")

I don't know who did it, but I know that there are a lot of people that would like to see her gone. She was stepping on to many toes for something not to happen to her.
