Try the book by Athul Kahate titled "Object Oriented Analysis and Design"

The book "Programming in C++", from Prentice Hall, ISBN:970-26-0254-8" fully covers UML basics with a C++ orientation. I recommend it a lot: only 30 bucks for about 1300 pages.
