Onscreen I wouldn't know -- because I am not that into the slasher movies, etc.

Implied on screen (aside from outright blowing up a planet... a la Star Wars: A new hope) maybe Damnation Alley (1977) where darn near the whole planet was decimated in a nuclear war.

I think LOTR (Lord of the Rings) would probably hold the record for the same stunt men being killed multiple times. In one of the special features the Orc/Uruk Hai stunt folks mentioned trying to count how many times he/she had been killed by Aragorn (Viggo Mortenson). He lost count after 50.... and there were roughly 60 main stunt personnel and three main fighting heroes (Aragorn, Legolas, and Gimli) -- you do the math.

It certainly seemed like Saving Private Ryan. Realistic body count too, and gory, but a good movie nonetheless. Star Wars, Scream, The Departed, Commando, Maximum Risk etc etc.

There was also the funny Sean of the Dead. High body count, and laughs too.

Here's a strange site that'll help you decide if the count is too high for your children!:

'TaeGukKi: The Brotherhood of War'
Fact: A total of about 25,000 extras fighting, killing, and dead on screen.

Highly emotional and realistic movie. Based on true story, war in Korea. A most excellent movie with exceptional acting, direction and cinematography. If you haven't seen it already, it's a must see.
