In the US most are dug by machines. I have even seen the bulldozer like things holding the dirt that will be put back over the casket parked not far from where the grave is dug. In places like Mexico I have seen people using shovels to dig and cover the graves.

If you live in the Appalachian Mountains as I do,...you do it by hand, with a dirt shovel. This is the one last respectful act you can do for the deceased and/or family of the deceased. You don't have to live in a third world country to see graves being dug by hand.

It depends on the cemetary's size. Most have cartakers or hire someone to get the grave ready, usually these days, by backhoes or similar diggers because of so many rules about concrete liners, etc. However, most often, if you hang around after the graveside interment service you will see someone with a shovel to put the dirt back on top and place the funeral bouquets on the top.(At least in my experience.)

Depends on where you live some smaller towns they are dug by shovel.
