Well, the saying is "I wouldnt piss on him if he were on fire" it is a contempt statement. But the piss on him if he were on fire would be the minimal effort to get water on the fire to put him out. It is a little cruder than the statement"I wouldnt give him the time of day." but it means the same thing,.

Meaning that I care so little for that person that I would not even psss on him to save him from burning to death?

the expesion is that they would not p1ss on someone if they were on fire. eg ( i hate you and wouldnt save your life if you needed it)
