It doesn't sound like he/she is doing anything wrong. As long as he keeping fair which means everyone have to follow the rules. In most jobs employers wont allow anyone to take a lunch break until four hours into their shift. Breaks usually are taken two hours into your shift and the second break at least 1 hour after you had your lunch break. There are some fast pace jobs that might not be able to give your break when you want it just like grocery clerks and fast food restaurants they need to serve the customers first and then give you break when they are able. If you think your employer is breaking any laws or treating you differently then talk to upper management if upper management don't do any thing check with your state office for Equal Employment Opportunity Commission.

If it's 9 or 10 AM then 1 or 2 hours would be reasonable. As to the bathroom breaks go on his desk and that should clear things up.

I have never heard of such a thing! Are you in the USA? I can't imagine any employer making you "apply" for permission to go to the bathroom? What if you are sick? Or pregnant?
If I were you, I would speak with the Department of Labor for your state.

I don't like the sound of this - You should make it plain that this is not acceptable. Speak to HR or if he is the one man HR department drop into conversation that you are considering taking him to tribunal for harrassment and bullying.

Federal law does not require lunch or coffee breaks.
Some states may have requirements for breaks or meal periods.

get policy in writing..
must be a policy stating what when where how ect.
cannot deny one bathroom usuage...
get that policy..if supervisor/employer is not complying contact state dept of labor...
