Upon the death of the ward, the guardianship/conservatorship ends. Period. Whatever property she had at death would become part of her estate, controlled by an executor appointed by the court. The property would be distributed to her her heirs (either by will or by intestacy). If the guardian/conservator was not an heir, they would inherit nothing.

Why would the living spouse have a guardian? The money would go to whomever her will names as a beneficiary. Also, she would probably have spent some of that 250k. I am guessing at this because the question makes no sense.
