She doesn't need anything. Clothes are hard to pick for her. She doesn't like perfume, candles, bath sets or anything that smells. She likes bike riding, but has a new bike. She goes sailing, but has everything for the boat that I can think of. She golfs, but not seriously, and has clubs. She does not like spas. Any ideas would be appreciated.

Tough one but I'll give it a try:
1) Years Subscription to a favourite magazine or other publication
2) Day at a health spa.
3) Night out with meal and theatre tickets
4) Portraint of herself. (most artists can work from photos and come across all price ranges)
5) Makover and photo session.
Hope something here gives you some food for thought.

A gift coupon from you for one days or as many as you like house cleaning or window washing or any job about the house that she doesn't like doing and would love to have someone else do for her. I made a coupon book by hand for my mom that expired within 60 days and she was a slave driver, LOL but really did love the idea.

What about taking her out to a nice dinner, and depending on where you live, some kind of show?
I have taken my mom to Broadway shows for her birthday many times, and she seems to enjoy it.
Plus you get the advantage of enjoying it also!
Or you could always go with an American Express gift card, so she can spend it on whatever she wants. They take them at most stores and restaurants.
