One voice of decent here, my understanding is that punctuation denoting the end of a sentence; full stop, exclamation marks and question marks can only be used at the end of a sentence, how do you do, in and of its self does not constitute a sentence and, therefor, would not have a question mark placed after it.

If you are asking a 'question' then you end it with a question mark. If it is a statement then it is a 'punctuation mark'!

I think so, because it is a question after all.

Some people ask it as a rhetorical question, but it still is a question, so requires a question mark.

Dictionary has it WITH the question mark, cos it's a question.

Just check the tone and mood when you ask someone the same- How do you do?
And sometimes you're reciprocated in the same manner - How do you do?
