Try formatting the floppy on your computer. Not all floppy drives are exactly the same, so sometimes a disk can work in one computer but not in another.
If that doesn't work, try putting some other floppy disks into your floppy drive and see if your computer can read them. If it can't then you have a problem with your floppy drive.
If your floppy drive used to work, and now it doesn't, then it is probably broken. Replace it. You can buy a floppy drive for $25.
You can also try booting off of the CD-ROM drive instead of the floppy. Insert your original operating system disk, turn the computer off, and then turn it on. If it doesn't boot from the CD-ROM, then modify your computer's BIOS settings (check your computer's manual or turn-on screen to see which function key you should hold down as your computer starts up) and add the CD-ROM to the BIOS' list of bootable devices.

You say you want to format your boot disk. You do know when you format a disk you erase any thing on it, right?
When using the 31/2 floppy drive, shut off the computer put the disk in the drive and then start the computer.
This should read the drive.

3 1/2" floppy drives made in the 80's were more reliable. In the 90's and later, they were flimsy and the loading mechanism wears out too easily. Unless you want to buy a new floppy drive, you'll have to keep re-inserting the disk until the disk manages to sit properly in the floppy drive so that it can be read.

it may be the laser on the disc could have been in contact with the sun or any light source and they may not allow the disc to work so the computer cant scan it as project as to bright to read u should try on a friends computer and see if it scans on there if it does then it is Ur drive that has the problem and it might say it is working properly but it could just be an error the computer hasn't picked up on

Floppy drive , magnetic head need to be cleans(dirty-stains),some floppy disc does not work or suitable with certain floopy drives,re-install your floopy drive

mmmmmmmmmm restart your comp hold down F8 bios will start ensure floppy seek is enabled F10/Y to restart be carefull u can stuff it
