A good main plot is essential. Preferably with a few surprise twists. A lot of freedom to wander about the world doing things unrelated to the main quest. Not just side quests, but fun stuff. A really good combat system that challenges your ability to think tactically and not just mash buttons or repeat the same attacks over and over. Smart enemies. A good leveling or skill advancement system that is flexible and rewarding. Good NPC interaction. Lots and lots of items. Humor. Drama.

Note that I did not list amazing graphics. That would be nice, but gameplay is more important than looking pretty. Oblivion is not considered the best RPG on the market just because it looks nice (although it sure does)

Flexible character creation is a must. Flexible character graphics would be even cooler. If someone were to make a great RPG with half the graphic character options of City of Heroes, it would be a huge advance.

A good character class system, a girl who doesn't dress like a wh*re but can still kick butt, characters who are fun but still have some depth, doesn't require hours at a time of pointless leveling to continue the plot. Oh, and a worthwhile plot, where there are actually surprises and humor. Plenty of sidequest, not just forced ones to make the game longer. And optional bosses that are worth while, both in difficulty and item drops.

A perfect balance of the following: intriguing weapons and items, an intuitive leveling system that makes you want to keep leveling up (along with awesome skills as you level), and a truly engrossing storyline. Add that to good voice acting, persuasive characters, non-repetitive enemies and dungeons, and you've got yourself a winner!

More mature themes. Most RPGs today are geared toward kiddies.
Like Final Fantasy. All the characters are like 14 years old.

revolutionary combat system with interactive environments.. uhh.. except with a pirate theme! Boats, falling overboard, struggling to survive and all that.
