A bit of advice -- you probably don't want to go into writing the article knowing what you want it to say when it's an 'investigative' piece. Why bother asking questions if you already know the answers, and you're only going to use them to fit your mold, anyway?
The reason you ask someone questions in an interview is to learn. You're supposed to go into it with an open mind, and you might find you've picked something up, or it's changed your mind.
Otherwise, that's 'agenda,' 'manipulative,' or 'biased' journalism.

How is the food inspected on arrival? How is it handled and by whom? How are the staff checked for staph infections or other possible infectious agents. Who is responsible for checking food temperatures during storage? What agency inspects the cafeteria? How often? How often is the floor and other surfaces steam-cleaned. What normal disinfectant is used for cleansing of tables, and other furniture. What temperature is the dishwasher set at? What disinfectant is used for the rinse of dishes and silverware. When frozen products are brought in, who is responsible for their immediate unloading and storage? What precautions are taken with fresh fruit or vegetables? Who recieves the reports of inferior sanitation and how often?

What instances lead you to believe the cafe isn't clean?
What is the worst thing you have seen or experienced in the cafeteria that made you believe it is not as clean as it should be?
Do you know where I can get a copy of a health dept. report? / May I see a copy of a health dept. report?
Those aren't raisins, are they?
Did some of the rice just move?

Have they ever had any health code violations?
Do they require food handler's training or certification?
What is the temperature in which hot foods are kept and cold foods are kept?
How often is the kitchen inspected?
Ask to see the kitchen. Take notes of anything you think is wrong, ask about those problems. Good luck.

Who prepares the food, are they required to wear gloves, hair nets, etc.?
How often and how quickly are spills cleaned up, tables wiped down, etc.?
How are the dishes, trays, silverware, etc., cleaned, are they just wiped off and steamed? Or do they actually use soap?
Those are the types of questions I'd be asking!
