It depends. Most of the time (e.g., if you aren't using a proxy), it's easy to determine at least the country (sometimes the state or even the town) of your ISP - and chances are that you live there, too.
Normally, you can't be located with such a precision as the address of your house - not by random user of the 'Net, I mean. However, a law enforcement officer can subpoena the records of your ISP - and *they* will have your address on record and will be able to match it with your IP number.

Well, if you have contracted out the ISP for a length of time then it can happen. However, most IPs are dynamic, and more importantly ISP will purchase them in blocks. So if someone knows your IP address, they cannot find out where you live. They can however get the information for your ISP, and if you have been doing illegal activities could eventually find this out. However that requires a court order for the ISP to release the personal information from their records. So no, you are safe. However just to be sure, you can go to the website below and enter your IP address and see what information it contains.

No, not just by an IP but if they have other types of information one site leads to another you are signed up for that gives a little more info than the last and pretty soon before you know it they have everything there is to know about you.
